Appreciate this Mopsy and sorry I was wrong.  Interesting that there is NO mention on the Vacunación site of needing this...... which of course should come as no surprise.

Got the 2nd shot today 🙏. Yes you require the original hand written receipt and the printout as well. They fill out the printed sheet less the data already there and take half of the sheet. One half to you the other they keep. They then will use this sheet to update the data file so that in about 2 week when you go online to print it off again all the info fields will be complete. Therefore they only have to input once.

Are you sure? My understanding is that you do not need your passport, just your visa. No one I know who got a shot needed to show a passport.

That only applied to people who do not have Permanente status and as a result do not have a CURP number.  That was the case for those of our friends when they received their first shot of Pfizer.  For those who have Permanente status their Permanente card was all they required, not their passports.
