Yes you require the original hand written receipt and the printout as well. They fill out the printed sheet less the data already there and take half of the sheet. One half to you the other they keep. They then will use this sheet to update the data file so that in about 2 week when you go online to print it off again all the info fields will be complete. That was the case for those of our friends when they received their first shot of Pfizer.
