There's, of course, no enclosed envelope and no address listed to send it to. We did finally get to the office in Guadalajara by email. They gave us an email address to send the scanned form to. It is as follows, fbu.guadalajara@ssa.gov as we were told several many times by the ssa in the States they had no way to help us. We were given phone numbers and addresses in Guadalajara that proved to be incorrect. I finally found the correct phone number and was told to go on line to make an appointment. They didn't make an appointment but handled everything by email. We downloaded the forms, filled them out and scanned them along with our passports and Permanente card, proof of address. This office gave a case number and all emails had to be in a chain ... A separate email to use generate another case number and cause much confusion. Hope this helps.
