You are expected to reply "Bueno, y Usted" well -and you? Usted establishes a sort of respect, if someone is well known to you, you might reply "Bueno, y Tu?" - best to stay away from that one at first. Well. You know you are a gringo, when someone, often a complete stranger, asks "how are you", a gringo spouts all their aches, pains and health problems. How friendly Mexicans are, by taking a personal interest in your health! Reminds of an old joke - that man is so boring, if you ask him how he is doing - he will tell you.
