As a Physician, I highly recommend that you make "END OF LIFE" and "POST LIFE" plans known to your doctor and have him/her document them in your chart.
Having recently had to deal with an END OF LIFE scenario I just want to remind everybody how it is such an emotional time for everybody and a patient's wishes should be clearly stated while of sound mind and body and then respected by his/her family, friends and loved ones.

I ask these questions during my initial office visit and then ask if there are any changes to the patient's wishes in subsequent visits
